To be sure that we're talking about the same device, I would refer my answer to the Philips hue Play HDMI Sync Box.
Basically, as you rightly suspect, the box is only intended for synchronizing external HDMI sources. Everything you play directly on the TV, whether classic DVB-C or newer VOD providers, is not covered by the box.
But if that's what you want, you can e.g. For classic television (ARD / ZDF / RTL / Pro7 / etc.) Put an appropriate receiver on the sync box. Such a receiver is e.g. Also the Horizon Box or the GigaTV Box, Magenta TV Stick / Box, etc.
For VoD content and providers, you can also buy appropriate devices such as an Amazon Fire TV or an Apple TV.

If the TV is able to show Netflix without a sync box, it can also use sync.
The sync box may not respond to Netflix.
