When I call a buddy and watch eg Youtube, I hear YouTube very softly and the caller loudly, not only YouTube is the same on netflix and all other sounds of the media volume. How do I get these louder during a call?

Not possible. Your mobile phone regulates the media volume exactly so that your conversation partner can't hear the sound.
