The last episode was more of a summary of events that took place in the anime, but not so important that they appear in the anime or in the main story. That means it is a sequence that shows what they have experienced in this world. It's not a sequel or anything else, after all, everyone was born again after the 13th episode. If you want to see the rest of the story I can recommend the Ova, because it goes on
